Upload your waiver
Web Owner · AdminAvex has built-in e-signature. You don’t need a separate waiver service.
Steps
Open documents
Go to Documents → New Document.
Upload your PDF or write it inline
Two paths:
- Upload a PDF — use your existing waiver as-is
- Build one here — write the document directly in Avex using the freeform canvas
Place your fields
For an uploaded PDF, drag fields onto the page where they belong: signature, date, printed name, initials, text boxes, checkboxes.
For each field set:
- Who fills it — the member, a guardian, or staff
- Required or optional
Set the requirement rules
This is what makes the waiver actually get signed:

Save and activate
What your members see
A member signing up on your public page hits the waiver before the payment step. They can’t skip it, and they can’t complete signup without it.
Existing members who need to sign a new document see a banner in the portal and the app that reappears every login until they sign.
Signing works on a phone — they draw with a finger.
Notes
- A member cannot reach active status without signing every required document. That rule lives in the database, not just the signup form, so there’s no path — including staff error — that produces an unsigned active member.
- Guardian signing is supported for minors. The signer’s relationship to the member is recorded alongside the signature.
- Every signature captures IP address, device, and timestamp, and generates a final PDF you can download.
- Versioning: editing a document creates a new version. Signatures record which version was signed, so your records stay accurate.
